There is no single nationally mandated template, which is why formats differ so much between companies. What a payslip does need is enough detail for someone else to verify it. In practice that means:

Provident fund is 12% of basic plus DA. Many employers apply the statutory wage ceiling of ₹15,000, which caps the employee deduction at ₹1,800 a month. The employer's matching contribution is part of CTC but never appears in the deductions column, because it was never part of gross pay.
Professional tax is a state levy of roughly ₹200 a month, capped at ₹2,500 a year. Several states and union territories, Delhi among them, do not levy it at all, so leave the line out entirely rather than entering zero.
TDS is the employer's estimate of annual income tax spread across the months, so it moves when a bonus lands or an investment declaration changes. It is not a fixed percentage of salary.
